Filling out your paperwork online prior to your appointment can save you time when you first arrive at the clinic. If you are unable to complete it online, please arrive at least 10 minutes before your appointment time.

In the event that you are unable to attend your appointment, we require a minimum of 24 hours notice. Failure to cancel before this time or not showing up for your appointment will result in a minimum fee of $50 being applied to your next appointment.

Booking a permanent cosmetic procedure requires a non-refundable deposit of $100 to be made in order to secure your appointment. Appointments are not considered to be verified until this deposit has been received.

If you are pregnant or nursing, please ensure that your procedure is safe to be performed on you. If you’re unsure, you’re welcome to contact your physician or our staff team to ask.

Due to the nature of the clinic and the services that we provide, we strongly recommend leaving young children at home.
